Member Activities

Our Member Services team is here to help you.
We can help you to:

  • Change your primary care practitioner if needed. Please remember that you will need to select a primary care practitioner who visits the nursing facility where you live. If you would like to change your primary care practitioner, you can also speak to your Longevity Health clinician for assistance.
  • Learn how to get a referral or authorization for covered services if needed. Please reach out to your Longevity Health Clinician any time to talk about your care. Your Longevity Health Clinician will work with you to arrange the care and services you need.

For example, if you need to see a specialist outside the facility, your Longevity Health clinician may work with you to schedule the appointment and arrange for transportation to the appointment if needed.

  • Learn about your benefits and financial responsibility for a specific service or treatment.
  • Learn about your benefits and financial responsibility from a specific provider or institution

For example, if you have questions about the cost of a specific treatment, the Member Services team can provide an estimate about possible out-of-pocket costs, if any.

The hours for the Member Services team are 8 a.m. to 8 p.m., seven days a week (except Thanksgiving and Christmas) from October 1 through March 31, and Monday to Friday (except holidays) from April 1 through September 30.
