High Level Case Management
All Members of Longevity Health are eligible for complex case management. The goal of complex case management is to help Members achieve their health goals.
Complex case management includes:
- An assessment of the Member’s condition
- Using available benefits and resources to support the Member
- Development of a care plan with goals, monitoring, and follow-up.
Your Longevity Health Clinician will be your single point of contact for the complex case management program. This Clinician will visit with you regularly in your facility to monitor your health and coordinate any medical appointments you may need.
Please reach out to your Longevity Health Clinician any time to talk about your care. Your Longevity Health Clinician will work with you to arrange the care and services you need.
Please note: You may opt out of the complex case management program at anytime.

- Members can communicate their desire to opt out during any encounter with LHP staff
