High Level Case Management

All Members of Longevity Health are eligible for complex case management. The goal of complex case management is to help Members achieve their health goals.

Complex case management includes:

  • An assessment of the Member’s condition
  • Using available benefits and resources to support the Member
  • Development of a care plan with goals, monitoring, and follow-up.

Your Longevity Health Clinician will be your single point of contact for the complex case management program. This Clinician will visit with you regularly in your facility to monitor your health and coordinate any medical appointments you may need.

Please reach out to your Longevity Health Clinician any time to talk about your care. Your Longevity Health Clinician will work with you to arrange the care and services you need.

Based on your health condition, you may be eligible for a higher level of case management. Please talk to your Primary Care Provider about your health care needs.

  • Members can communicate their desire to opt out during any encounter with Longevity Health staff
