FIND A PROVIDER

To search for providers, enter a value into any search field or combination of search fields, and then click on the “Search” button to view the results.

For example, you may search for a provider by Specialty, Zip Code, and Languages Spoken by entering values into these fields and clicking the “Search” button.

To search for a hospital, first select Hospital as the Provider Type. The Hospital Name field will appear. You may enter the hospital name in this field to search for a hospital by name.

You may also search for a hospital by location. To search for a hospital by location, enter the zip code and mile radius, and then click on the Search button to view the results. If you would like a PDF version of the entire network within a state click “Provider List” below.
