PROVIDER NOTIFICATION – PRIOR AUTHORIZATION SUBMISSION
In January of 2026, Longevity Health will be transitioning to a new provider portal for authorizations. Information will be posted to www.longevityhealthplan.com when available to provide a step-by-step process on how to enroll and access the provider portal used for prior authorization submission.
In the interim, we invite you to submit your authorization request via fax using the attached form. Completion of this form will ensure all mandatory and required fields are captured ensuring smooth processing of your request. Please note that any fields on the form not completed or left blank may impact the ability of our Utilization Management department to process your request.
