Prescription Drug Benefits

Longevity Health Plan (HMO I-SNP) of Illinois provides Medicare Part D prescription drug coverage through our partner Navitus Health Solutions, a full-service pharmacy benefit management company.

Stage 1 Yearly Deductible: Your yearly deductible is $445 for all Part D prescription drugs
Stage 2 Initial Coverage: After your yearly deductible is met, you pay 25% coinsurance for all drugs covered by this plan until your total yearly drug costs reach $4,130.
Stage 3 Coverage Gap: After your total drug costs reach $4,130 (including what our plan has paid and what you have paid), you will pay no more than 37% coinsurance for generic drugs or 25% coinsurance for brand name drugs, for any drug tier during the coverage gap.
Stage 4 Catastrophic Coverage: After your yearly out-of-pocket drug costs reach $6,550 (including drugs purchased through your retail pharmacy and through mail order) you pay the greater dollar amount of either 5% coinsurance, or $3.70 copayment for generic (including brand drugs treated as generic) and a $9.20 copayment for all other drugs.

If you take many medications for more than one chronic health condition you may be eligible to participate in the Medication Therapy Management (MTM) program offered by the Centers for Medicare and Medicaid Services.

Cost-sharing may differ based on point-of-service (retail, Long Term Care (LTC)), home infusion, whether the pharmacy is in our standard network, or whether the prescription is a short-term (30-day supply) or long-term (90-day supply).

What is an I-SNP and who is it for?
I-SNP is a type of Medicare Advantage coordinated care plan specifically designed to meet the needs of individuals living in a Long-Term Care facility or Assisted Living facility. I-SNP requires the person to be eligible for Medicare Part A and B, live in or be deemed as needing an institutional level of care and reside in a contracted facility.
Am I eligible to join Longevity if I have Medicaid?

Anyone over 18 with Medicare Part A and Part B, who lives in a participating facility and is receiving nursing home level care, can enroll in ISNP. Individuals with Medical Assistance (Medicaid) are also eligible to enroll, however, dual eligible individuals are strongly encouraged to review the benefits of your state, which offers integrated support for individuals on both Medical Assistance and Medicare with very little, if any, out-of-pocket costs.

What benefits does Longevity offer?
Benefits are determined based on the state you receive services. Some examples include: Advanced Practice Provider (APP) as a dedicated clinical resource; dental; vision; medical transportation; therapeutic music sessions; access to an Over the Counter (OTC) medical supply catalog; and more based on your market.
When can individuals enroll?
People may enroll anytime throughout the year, with a start date the 1st of the month after enrollment is completed.

Can I keep my primary care physician?
Yes! We work hard to ensure our provider network is robust and members can stay with their existing PCPs if they prefer. Our team collaborates with its members’ additional health care providers for comprehensive care and support.
Can I get care from a specialist?
Yes, you can get care from a specialist; for Longevity to cover a specialist visit, you must see a specialist participating in our network. You do not need a referral or authorization from your Primary Care Provider (PCP) to see a specialist.
